Community HealthChoices Fact Sheet: ODP Service Providers
Community HealthChoices (CHC) is Pennsylvania’s mandatory managed care program for individuals who are dually-eligible for Medicare and Medicaid and individuals 21 years of age or older with physical disabilities.
Individuals with an intellectual disability or autism who receive services beyond supports coordination through the Office of Developmental Programs (ODP) will not be enrolled in CHC.
However, if an individual needs a level of care provided in a nursing facility, they may choose to transition into CHC.
