CRAWFORD COUNTY INTELLECTUAL DISABILITIES
The Intellectual Disability Department of Human Services provides Support Coordination services for individuals with Intellectual Disabilities who live in Crawford County.
Intellectual Disability is defined as having a below average level of intelligence and significant limitations in major life activities such as learning, language skills, and the capacity for independent living.
These conditions usually occur during the developmental period of a person’s life, which occurs before they reach 22 years of age.
All individuals will be assigned a Supports Coordinator (SC) who will assist in locating, coordinating, and monitoring services and supports. The SC will complete an Individualized Service Plan (ISP) with the people who have an important role in the individual’s life. This plan will outline the individual’s likes and abilities, as well as their goals and desired outcomes in life. The SC will complete a Prioritization of Urgency of Needs (PUNS) form, which will assist the individual and their family in planning for future needs.
